The Children's Doctor is a medical family which provides quality healthcare services to infants, children, and youth in the Tri-State area. We strive to enhance the health and well-being of all children through caring, respectful, and efficient patient care.

Patient Forms
These are handouts which offer the convenience of answering questions prior to your visit. When you click on the link you will have the option to open the file or save it to your computer, you can choose either. You have 3 options on how to use them.
- You may fill it on your computer and e-mail it to us ahead of time. After filling the form, left click "file" on the upper left corner of your page, select "save as" and save the file to wherever you choose (do not try to directly send it from the "file" menu, it will not work correctly). Then, send the file as an e-mail attachment to the following address: [email protected] . Keep in mind that you will need to sign it later in the office. One word of warning, to fill it online you will need to have Microsoft Word on your computer, otherwise it's best to go on to option #3 below.
- You may fill it on your computer, print it out, and bring it to your next visit. Again, you would need Microsoft Word to do this.
- You may print it out, hand fill it, and bring it to your next visit.

What to expect
A pleasant, comfortable first visit builds trust and helps put the child at ease during future visits. We want your child to enjoy getting to know our doctors and staff, so we work hard to establish this bond during every appointment to our office. During your initial appointment, we will explain everything in detail and answer any questions you may have.
